What this site publishes
What employers' benefit plans reported about their insurance contracts on a public federal form. A broker or agent is named only when the name is clearly a firm's; an individual agent appears as “(individual agent)”. The Form 5500 also carries the names of signers and plan administrators and phone numbers: none is republished. No information about any employee or covered person exists in these files or on this site.
What your browser sends
No analytics, no advertising, no cookies. Searches run in your browser against static files. A click on a referral link is counted (site, link and page — no visitor identifier). The host (Cloudflare Pages) keeps ordinary server logs.
How we are paid
- Dated reports and Pro access.
- A referral: some pages and records carry a link to Registres, an independent data gateway that introduces companies to AI labs licensing internal workflows. This site may be paid a fee if a company that applies through the link is accepted. No one pays to appear here.
Corrections and removal
The data is a public federal record. If something is wrong, if a personal name slipped through, or if you want an entry removed from this site, write to us: we cannot change the federal filing, but this site is not the government.
