What this is
An employer whose health and welfare benefit plan files a Form 5500 attaches a Schedule A for each insurance contract: the carrier, the benefits, the persons covered, the premiums paid, and the commissions and fees paid to brokers. The Department of Labor publishes these filings as large files. This site reads Schedule A — the part other Form 5500 sites leave out — and gives every employer a record you can find, read and cite.

What it is not
Not the Department of Labor, and not a quote. Only plans that file a Form 5500 with a Schedule A appear — most plans with fewer than 100 participants do not — and a self-funded plan reports only its insured pieces, such as stop loss. How to read the records.
